Annual Declaration of Past Activities Guide

You have been involved in the previous calendar year in producing, processing, consuming, acquiring, supplying, or storing any chemical listed in Schedule 1 in any quantity and in any concentration.

Schedule 1 Annual Past Declaration Questions

Company / Institution
Name of company or institution completing the form. The company / institution operating the plant site or facility dealing with the Scheduled chemicals should complete and submit declarations. The Canadian National Authority (C.N.A.) must receive the declarations from the operators of such plant sites or facilities. These operators may or may not be the actual owners.

Street Address
Address of the Canadian headquarters or head office of the company / institution.

City
City of the Canadian headquarters or head office of the company / institution.

Province
Province of the Canadian headquarters or head office of the company / institution.

Postal Code
Postal Code of the Canadian headquarters or head office of the company / institution.

Contact Name
The person within your company / institution that should be contacted by the Canadian National Authority if necessary.

Contact Title
The title of the person listed as the contact.

Phone
The telephone number with area code, of the person listed as the contact.

Fax
The facsimile number with area code, of the person listed as the contact.

E-Mail
The internet e-mail address of the person listed as the contact.

Plant Site / Facility
The local integration of one or more plants, with any number of intermediate administrative levels, which are under one operational control and which include a common infrastructure.

For universities and research institutes, the plant site would be the main building housing the laboratories.

Please indicate the name of the operator of the plant site / facility.

Name of owner
If the operator of the plant site / institution is not the owner, please indicate the name of the owner.

Address of the plant site / facility.

Types of Schedule 1 Facilities

Single Small-Scale Facility (SSSF)
This designation only applies to the government facility located at CFB Suffield which is used for protective research.

Protective Faciliity
Canada is allowed under the CWC to have one other facility aside from the SSSF to conduct protective research. Such research is for purposes directly related to the protection against toxic chemicals and to the protection against chemical weapons. Canada has not designated this facility.

Facility for research, medical or pharmaceutical purposes
This is the category for most facilities in Canada that will be dealing in any way with Schedule 1 chemicals.

Import Export company
This is the category for all facilities in Canada that will only be importing or exporting Schedule 1 chemicals.

Exact Location
Identify the location where activities involving Schedule 1 chemicals have taken place (building, structure number). Exact location of the production, processing, consumption, acquisition, supply or storage of the Schedule 1 chemicals, in the previous calendar year.

How many Schedule 1 chemicals have you been involved with, in any capacity, in the previous calendar year?

How many Scheduled chemicals were used as precursors for the above Schedule 1 chemicals?

How many times did you receive or supply a Schedule 1 chemical in the previous calendar year?

IUPAC chemical name
Use the International Union of Pure and Applied Chemistry (IUPAC) formal system of nomenclature to identify the Schedule 1 chemical. Do not use common or trade names in place of the IUPAC name unless they are specifically listed in Schedule 1.

CAS registry number
Use the Chemical Abstracts Service code number, if assigned, to identify the Schedule 1 chemical.

Quantity produced
Total quantity of this Schedule 1 chemical produced (or acquired from natural sources i.e. toxins).

Quantity consumed
Total quantity of the Schedule 1 chemical consumed. Consumption of a chemical means its conversion into another chemical via a chemical reaction.

Purposes of consumption
Identify the purpose of the consumption of the Schedule 1 chemical.

Domestic quantity received
Aggregate total of the quantity of the Schedule 1 chemical received from all other facilities in Canada. Only to be completed by the SSSF.

Domestic quantity supplied
Aggregate total of the quantity of the Schedule 1 chemical supplied to all other facilities in Canada.

Maximum quantity stored
Maximum quantity of the Schedule 1 chemical stored at any time during the previous year.

Year end quantity stored
Total quantity of the Schedule 1 chemical held or stored on December 31st of the previous year.

Precursor IUPAC Name
Use the International Union of Pure and Applied Chemistry (IUPAC) formal system of nomenclature to identify the Scheduled chemical precursor. Do not use common or trade names in place of the IUPAC name unless they are specifically listed in a Schedule.

Precursor CAS registry number
Use the Chemical Abstracts Service code number to identify the Scheduled chemical precursor.

Quantity of precursor
Total quantity used of the Scheduled chemical precursor.

Received / Supplied
Indicate whether the chemical was received or supplied. Please remember that you must detail each occurrence individually.

Country Code
See country codes

If the transfer is domestic, enter CAN for Canada. Record each occurrence that your company / institution supplied a Schedule 1 chemical to another company or institution or received a Schedule 1 chemical from another company or institution.

Name of company / institution
Indicate the name of the company / institution that received from you or supplied to you.

Purposes of consumption
Identify the purpose of the consumption of the Schedule 1 chemical.
